HUD Grant To Fund Service Coordinator for Local Seniors

Heritage Tower, 1145 College Ave.,
has received a grant from the United States Department
of Housing and Urban Development (HUD) to provide for
a service coordinator to serve the older adult population
at the independent living facility. The grant allows
for
nearly $180,000 over a three-year term. This allows
current staff, already providing services, to join a national
effort to provide service coordination that follows
a high standard
of practices regulated through defined quality assurance
standards. Heritage Tower, along with 124 other grant
recipients across the nation, will work to recognize
the needs of
the elderly residents and coordinate necessary services
and interventions.
The Heritage Tower service coordination program will
be managed by Heritage at Home, a program within the
Wexner
Heritage Village family of services that aims to help
older adults continue to live independently in the
community. At the end of the initial term, Heritage
at Home may
apply
for renewal to continue the program.
Wexner Heritage Village is a not-for-profit provider
of health, housing, social and spiritual services
in the central
Ohio community, primarily serving older adults and
persons with disabilities. Its mission, guided by
Jewish values
and traditions, is to assist those it serves in pursuing
their fullest human potential. The campus serves
the entire community and includes: Wexner Heritage House,
a 200-bed
nursing facility, Heritage Tower and Bexley Heritage
Apartments, HUD-sponsored independent living, Creekside
at the
Village,
premier independent apartments, Victor Weinstein
Shalom
House and Shalom House South, two MRDD group homes,
Village Shalom Apartments, Heritage at Home, and
the Larry & Leonore
Zusman Jewish Community Hospice.
